I HEREBY ACKNOWLEDGE that I have voluntarily applied to participate in this skiing/party activities sponsored, sanctioned, approved and organized by the Scottsdale Sea &Ski Club (hereinafter referred to as “SS&SC”).
AS LAWFUL CONSIDERATION for being permitted by SS&SC to participate in skiing/party activities, I hereby RELEASE AND DISCHARGE SS&SC from all actions, claims or demands that I, my heirs, distributees, guardians, legal representatives, or assigns now have or may hereafter have for injury, death, or damage resulting from my participation in any activities.
I understand that photographs may be taken by members and others at this event.
I have read the Cancellation & Refund Policy of the Scottsdale Sea & Ski Club and agree to the terms and conditions thereto.
